In order to get started, we will need the following from you:
- Documentation of your federal or state operating authority – MC or DOT #.
- A copy of your cargo and automobile liability insurance, naming Tripoint Logistics as a certificate holder, issued by your insurance company.
- A completed W-9 or W-8 form.
